U=U stands for Undetectable = Untransmittable.

It is one of the most important scientific breakthroughs in the history of HIV — and one of the most misunderstood.

Here is what it means in plain language: when a person living with HIV takes antiretroviral therapy (ART) consistently and their viral load becomes undetectable, they cannot sexually transmit HIV to another person.
Not "less likely to." Not "reduced risk of." Cannot.

This has been confirmed by large-scale clinical studies involving tens of thousands of couples across multiple countries. It is the current scientific and medical consensus. It is not a theory — it is a fact.

And yet many people, including many people living with HIV, have never heard of it.

That matters — because U=U changes everything. It changes treatment goals, it changes conversations between partners, and it changes the meaning of a positive diagnosis. It is also one of the most powerful tools we have against HIV stigma.

How does U=U work?

What is a viral load?
Viral load refers to the amount of HIV present in a person's blood. When someone acquires HIV and is not on treatment, their viral load is high — meaning there is a large amount of the virus circulating in their body.

What does antiretroviral therapy do?
Antiretroviral therapy (ART) — daily HIV medication — works by suppressing HIV replication. Over time, it reduces the amount of virus in the blood to levels so low that standard tests can no longer detect it.

This is called an undetectable viral load.

Why does undetectable mean untransmittable?
Sexual transmission of HIV requires a sufficient quantity of the virus to be present in bodily fluids. When viral load is undetectable, the amount of virus present is too low to be transmitted during sex.

No virus to transmit = no transmission. That is U=U.

THE EVIDENCE

What does the research say?

U=U is backed by three major international clinical studies:

HPTN 052 (2011, updated 2016)

Followed 1,763 serodiscordant couples (one partner with HIV, one without). There were zero linked HIV transmissions when the HIV-positive partner had an undetectable viral load.

PARTNER study (2016)

Tracked 1,166 couples over 58,000 condomless sex acts. Zero transmissions from partners with undetectable viral loads.

PARTNER2 (2019)

Extended the PARTNER study to focus specifically on gay and bisexual men — the most underrepresented group in earlier research. Over 77,000 condomless sex acts. Zero transmissions.
Combined, these studies provide overwhelming, peer-reviewed evidence that U=U is real,
 reliable, and applies to everyone.

What U=U means in real life

If you are living with HIV
U=U means that getting on treatment and staying undetectable protects the people you love.

It means your HIV status does not have to define your relationships, your intimacy, or your future.

It means that the goal of HIV treatment is not just to keep you healthy — it is to make transmission impossible.

It means you have the right to share this information with your partners, your family, and anyone who has ever made you feel like a risk rather than a person.

If your partner is living with HIV
If your partner is on treatment and has a confirmed undetectable viral load, the scientific evidence is clear: there is no risk of acquiring HIV from them through sex.

This does not mean stopping conversations about sexual health. It means having those conversations with accurate information — not fear.

U=U reduces stigma. It challenges the idea that people living with HIV are dangerous. It replaces fear with fact. And it gives people a reason to get tested, get on treatment, and stay in care — because the benefits extend not just to themselves, but to everyone around them.

What U=U does and does not cover

U=U applies to:

Sexual transmission of HIV between partners when the HIV-positive person has a confirmed, sustained undetectable viral load

U=U does not cover:

  • Sharing needles or injection equipment — transmission risk through this route is separate
  • Mother-to-child transmission during pregnancy, delivery, or breastfeeding — speak with your care team
  • Periods when viral load may temporarily rise above undetectable (called "blips") — consistency with medication matters
  • Other sexually transmitted infections — U=U is specific to HIV. Condoms continue to protect against STIs
U=U requires consistency. Missing doses can allow viral load to rise. The protection U=U provides depends on staying on medication as prescribed and having your viral load confirmed regularly through testing.

GETTING TO UNDETECTABLE

How do you become undetectable?

Becoming undetectable starts with starting treatment — and most people on antiretroviral therapy reach an undetectable viral load within 3–6 months of beginning medication.

There is no single "undetectable" number — labs define it differently, but typically a viral load below 20–50 copies per milliliter is considered undetectable. Your provider will explain what your specific results mean.

Questions we hear most often about U=U

U=U means there is no risk of HIV transmission when viral load is undetectable. Condoms continue to protect against other sexually transmitted infections. Whether to use condoms is a personal decision — one to make with accurate information and open communication with your partner.
Most people on stable HIV treatment have their viral load tested every 3–6 months. Your care team will set a schedule based on your individual situation.
Occasional temporary rises ("blips") can occur. This is why consistent medication and regular monitoring matter. Your care team will work with you if this happens.
U=U has been studied specifically in the context of sexual transmission. The evidence is consistent across vaginal and anal sex. Talk to your provider if you have specific questions about your situation.
